Registration

On the hyperlinks below, you will find all the forms required to register your child at Bow Elementary School. In order for a registration to be processed all documents must be completed and turned into the BES office. You can bring or mail your documents to:

REGISTRATION CHECKLIST:

	Completed REGISTRATION form
	Copy of BIRTH CERTIFICATE
	Completed HEALTH SERVICES form
	Completed STUDENT HEALTH HISTORY form including immunization record
	Completed & signed RESIDENCY DECLARATION
	PROOF OF RESIDENCY (Requires two of the following documents: Utility bill indictating legal residence, lease agreement or rent receipt indicating legal residence with the landlord's address and telephone number, documentation of home ownership in the town of Bow.)
	Completed DEVELOPMENTAL HISTORY form
	Copies of all Legal Documents/ Court Orders (parenting plan and custody guidelines, guardianship, restraining orders, orders with regard to educational records)
